What to Bring to Your Appointment
Please remember to bring the following to every appointment:
- Photo ID
- Insurance cards and information
- Insurance co-payment
- All current medications in their original container OR a list of all medications with dosages.
- List of supplements or over the counter remedies
- You may need to bring a Referral from your Primary Care Doctor to see a specialist
